Bar and Lounge, second floor

Go behind the counter, to the right of the flower arrangement. Left 10, Right 80, and Left 30 get you another three coins. 

Insanity swaps this one to Right 20, Left 50, and Right 30. It is also the safest and most often quoted wrong online, so check which column you are reading before you start spinning. 

East Wing Waiting Room, the free one

Nothing to solve here. The safe behind the reception desk already stands open and holds two antique coins on casual and standard. On Insanity it stays open and sits empty, so skip the trip. 

Basement, the forklift room

Down in the inspection area near the workshop, a small tool room sits beside a forklift. Right 60, Left 40, and Right 80 get you handgun ammo and the Raccoon Roundup Map, which marks every Mr. Raccoon in Rhodes Hill. Leave it locked, and you hunt those collectibles blind. 

The two ARK safes

Both late safes live in the underground ARK facility. Neither is optional if the trophy matters to you, and the area gates behind story progress, so clear them while you are down there. 

Sterilization Chamber

Look for the umbrella storage crate on the counter below the window, just before you step into Bioweapon Repository 05. The combination is Right 10, Left 90, and Right 20. A Med Injector, a Molotov Cocktail, and 15 handgun rounds come out, which counts as a real resupply at that stage. 

Monitor Control Room

Two orbs get you through the door. Once inside, the safe waits on the desk at the far end of the middle-right container. Left 50, Right 60, Left 80 hands over a Med Injector, a Bottle of Acid, and an Empty Bottle. 

What Insanity Actually Changes

Insanity becomes selectable once you finish the campaign. It is far more than a damage multiplier. 

  • All five combinations change, exactly as listed in the table above. 
  • Antique Coin placements move, and some coins now drop from defeated enemies. 
  • • Key items relocate, including Grace’s B934 handgun and the R.I.P. knife. 
  • Saving requires Ink Ribbons, capped at 25 across the whole run. 
  • Aim assist switches off, and downed zombies can mutate back up. 

So a standard coin route will not carry across. Neither will a standard code list, which is the trap that sends people back to a search bar halfway through a run.

The padlock outside the Lead Researcher’s Office takes no code whatsoever. Pour the corrosive acid from the conference room over it, and the thing dissolves.
